Building a High Performance Team

Pre-condition:


Back on Track and Ambulatory Care are outpatient rehabilitation teams funded by different sources at a Toronto hospital. Despite treating very similar patients, these teams work separately and independently of each other with no desire to come together. In fact, they believe merging would be detrimental to the patient and to their own team.


Issue:


The hospital is building a new wing, which will house the new rehabilitation department and the two teams must come together and share a single point of patient entry, space, and equipment.


Both teams are very resistant to this change and have become difficult to work with.


Objectives:


Bring two outpatient rehabilitation teams together to examine possibilities and opportunities for working together. Create one vibrant team that plays and works together effectively.


Results:


Even before the new wing was built, the teams began planning ways they could work together to fulfill the future they created. They immediately established groups to look at the various processes and systems that would need to be in place to support the merged teams. Teams are now collaborating and supporting each other with great enthusiasm and excitement to the benefit of their patients.
